Payment authorization

I, the customer named above, authorize Layby USA to check my card today for $0 — nothing is charged now — and then to charge my card:

• — payments of — every week, starting — and ending —.
• Total of all payments: — — the sticker price. No sales tax is added — the price I see is the price I pay. Nothing more, ever.
• My order ships within 20 business days after my final payment — free.
• I can cancel anytime before dispatch. If I cancel — or if the plan is closed after 14 days' notice for missed payments — a cancellation fee applies: 15% of the total purchase price, capped at $150. It can never be more than I have paid, it is only taken from payments I have already made, and the rest goes back to my card.
• I am never obligated to complete the payments. If a payment fails it may be retried; if the installment finally fails it is marked missed and a $3.50 administration fee is added to my balance. Those fees are only ever taken out of money I have already paid — they can never leave me owing anything.
• Charges appear on my card statement as LAYAWAYUS*.
• A copy of this authorization will be emailed to you.
